Crossroads Sports 2025

By completing and initialing this form I, the parent/guardian of the above-named child, give permission for the above-named child my to take part in the Crossroads Sports League operated by Crossroads Baptist Church and its volunteers. I, the parent/guardian of the above-named child, assert that to my knowledge the above-named child is physically fit enough and able to participate in all athletic activities which may be directly or indirectly sponsored by Crossroads for the Crossroads Sports League. Should my child require immediate or emergency medical care while engaged in an activity sponsored by the Church, in my absence, I hereby grant the Church authority to release my child for medical treatment to such medical personnel as the Church determines appropriate under the circumstances. In consideration for the privilege of allowing my child to participate in the above-named activity, I agree to release and hold harmless the Church, its officers, agents, and volunteers from any liability to or responsibility for bodily injury, damage, illness, or death of the above-named child while participating in any athletic or social activity which may be directly or indirectly sponsored by Crossroads during this activity. Further, I agree to indemnify and hold harmless Crossroads, its officers, agents, and volunteers with respect to any claim asserted by or on behalf of my child as a result of bodily injury, illness, damage, or death.
